Enrolment Forms

OHSC are a separate legal entity to school, so must have our own enrolment information stored and available to staff at OHSC. Download the forms below:

1. ENROLMENT FORM:

MANDATORY FORM to submit prior to commencing
One per child in each family
Attach immunisation record
If your child has a medical condition download the form below

2. EZI-DEBIT FORM:

MANDATORY FORM
ALL fees are paid via direct debit every fortnight
This form sets up your direct debit

3. MEDICAL CONDITIONS FORMS:

If you have any medical conditions you are required to complete a Risk Minimisation and Communication Plan. For more information please refer to the medical conditions page
